Sample Personalized Safety Plan

Written by Casey Gwinn, and adapted by Barbara Hart and Jane Stuehling in 1992, the Personalized Safety Plan is a sample document which can be used to tailor the steps for increased safety to any individual. It includes information regarding safety during a violent incident, safety when preparing to leave, safety at home, safety with a protection order, safety on the job and in public, safety and emotional health, items to take when leaving, and telephone numbers to know.
